My name is Brandon Styll and I am your host. We are powered by Gordon Food Service and today is a continuation of Mental Health Awareness Month. It is also Men's Health Awareness Month and our guest today is Dr. Joe Pizzona, MD. He is a board certified urologist and today we're talking about all things men's health. Last episode we talked about mental health and I shared a lot about men's mental health and today I want to talk about women's mental health and I have a special guest, my lovely wife Jennifer. Say hi Jennifer. Hi. So let's start off and let's talk a little bit about some stats that we have for women's mental health. About 23.4% of adult women in the U.S. experience anxiety disorders in the past year as compared to 14.3% of men. Depression is approximately twice as common in women as in men with 24.5% of women showing depression symptoms versus 18% of men. Of those with depression, 43% women received counseling or therapy in the past year versus 33% of men. Great job women. Great job going to get some counseling. I love that but more broadly 57% women with any mental health issues get treatment annually compared to 40% of men. Again that's a great stat.
03:48Wonder why women live longer hun? Because y'all go and get help. I think we're really affects women in a big way and we're gonna get in something with you here in just a moment but one in five mothers experience maternal mental health issues like postpartum depression or anxiety each year and 75% of those affected women go untreated which is really really tough. A recent survey found 73% women feel they carry the stress of loved ones daily and only 18% are very open about that struggle. On average women report feeling stress for five hours per day with younger women, Gen Z and Millennials, reporting six hours of stress per day. 60% believe in women believe being a woman in 2025 is more stressful than ever. Why this matters? These numbers highlight the higher burden of anxiety and depression among women, the critical treatment gap especially for maternal mental health and the often unseen emotional labor that amplifies stress and I was looking at my analytics from like who listens to the show my number one demographic is between 31 and 45. That's a pretty big range that's women it's and it's 40% women listening in that age demographic which you Jennifer fall into that age demographic and I wanted to talk about something that a lot of people fully understand and that's perimenopause. I asked Dr. Pezone about it later on in the episode but this is something that you are uniquely experienced in. Unfortunately yeah. It is a thing so tell us about perimenopause and what has meant to you. Oh well I cruised into 40 I was like this is great feeling great felt fine and then a few months before
05:49my 41st birthday my anxiety which I'm anxious baseline you know that but kind of went through the roof and just menstrual irregularities and things like that and immediately I started thinking like this feels like there's something hormonally off and of course I scheduled an appointment with a doctor and there that wasn't even on their radar they just wanted to do a lot of different tests and things like that not test my hormones which was really frustrating because that's the first thing I asked for and was immediately denied. Well I want to tell you when we get in this episode Dr. Pezone one of the things he talks about is insurance companies and how they don't do insurance and if you're a doctor what they when insurance companies pay you for is to do all of these tests so when you come in if you go to a doctor that takes insurance they're gonna go oh we need to do 15 different tests on you but those are just tests so that they can have them done to get paid not tests that they're listening to you finding out what you're doing so that you can get actual help.
06:54Right yeah if they would have listened to my symptoms and you know I understand there are things that they need to rule out in order to give a diagnosis but seems like a good baseline to start me being of almost 41 year old female I'm in the perfect age to start perimenopause seems like hormones would be a logical choice to check first but that is not to the first line of treatment unfortunately for most physicians so. So you found a different physician. I did and then I had to find a different position and a different position so. It's been a battle to find somebody who will stop and listen to you. Yeah I had to end up I ended up going like a more holistic route so I knew I would get my hormones checked that way and it was a self-pay which most of them are so but you know up front that's kind of. So perimenopause is something that happens with two women between the ages of 35 and 46 ish but ten years but eight years before menopause actually begins which they say is between 51 and 55 for most women. Eight to ten years of perimenopause yes. But this is a lot of our listeners right now who may be experiencing these hot flashes or trouble sleeping or just a bunch of different symptoms and if you get in some of the symptoms here for perimenopause hot flashes night sweats they affect 35 to 50 percent of women in perimenopause. Vassa motor symptoms hot flashes night sweats palpitations occur in 75 to 80 percent of menopausal transition cases fatigue is surprisingly common affecting over 54 percent following by dizziness insomnia dizziness fit 44 percent insomnia 40 percent headaches 39 percent and joint pain. Now I think a lot of us work in restaurants we're like yeah all those things happen to me every day but if they are increasing and that's something that you're like this isn't a normal level mood changes such as irritability low energy tearfulness brain fog brain
08:55fog is a big one I mean we'll be talking you like what what's going on right now can't remember anything anymore it's very frustrating also like the fatigue that was another like big red flag for me like I would have just loved to have laid on the couch all day long and that was really not my character so that was really frustrating it couldn't get off the couch. And it's widespread like all women go through this but it's very under recognized like they don't talk about it it's not something that is a major topic of conversation and it happens to like literally every woman but yeah maybe not to this degree but everybody goes through premenopausal situation the situation right? Yeah I mean the symptoms are these are just a few I fortunately haven't experienced any night sweats or hot flashes yet but I mean itchy ears frozen shoulder like the craziest things that just you wouldn't really necessarily connect the dots to you know perimenopause or menopause so yeah it's it's kind of all over the place the brain fog is so so frustrating. Well there you go there that's I don't know what more to say on it but I know these are things that you're experiencing and me being a dude I can talk about men's health and I can talk about my own struggles but I know for a lot of women out there that may having some of these symptoms hearing it come from a woman that hey look these things are real and you've got to get out there and fight for yourself. You do if your gut is telling you that it's not you know just whatever your doctor's telling you and for me I knew I wanted to be on hormone replacement therapy and that has helped so much I can't say enough good things about it I'm off the couch and I'm moving around and I can't say my brain fogs great but I mean I feel a lot better than I did when I first started and so I don't know anybody who moves as much as she's like 20,000 steps a day she's all over the
10:58place you don't stop ever and fatigue will really that's just the way you've always been and so it's really easy for you to tell like oh wait a minute why am I feeling so tired but you've powered through it and I'm so proud of you and I think that you're amazing and I love you all right well we are going to get in to this episode right now with dr. Joe Pizzona to Nashville Restaurant Radio how you doing doc I'm doing great Brandon thanks for having me I am so excited to have you here it is men's health awareness month and it is also mental health awareness month and I think that in this industry we have a lot of people that focus on getting stuff done in their building and we don't stop to focus on ourselves and I really wanted to bring you in because I have a million questions for you and I wanted to bring some awareness around these topics no I think I think that sounds great as a small business owner myself and busy entrepreneur father husband I get it you know so I my brother actually has worked in the restaurant industry so I have a little bit of a taste for what some of your listeners may be battling and then literally a taste because I love going out and eating so I see how much hard work goes into forming a restaurant and keeping it going well let's talk about what you do and some of your credentials to kind of just say hey this is who dr. Joe is you're the owner of Pizzona MD tell us about Pizzona MD so we're a medical practice where we help patients optimize their performance in the bedroom the bathroom and the board room now what does that mean exactly so I am a board certified urologist which means I did a six-year training program after medical school to specializes in diseases of the urinary tract in men and women as well as the male genitalia the the junk down there yeah and what I realized throughout my career was almost every disease I treated had a root cause in lifestyle what we ate whether we slept how stressed we were were we overweight every disease from erectile dysfunction to infections to even prostate cancer came down to the waistline and I'm not the sort of person who's real comfortable just pushing pills saying here you go get out of my office yeah so ultimately back in January we
15:02opted out of insurance and opted in with our patients so we're now a direct care direct payment model which is very simple you pay me I deliver medical services if you're happy I'm happy and we've cut out the middleman does that make it more expensive or less expensive well that's a great question it really depends on what your insurance coverage was like beforehand right and that if you are being funded by the state or the federal government where health care was quote free and you've paid in then it's more expensive if you're someone with a high deductible plan with God knows who knows what your deductible is that's me right it can actually be more cost effective but the more important thing is we're cutting out the middleman and we can now put a value to our services that's commiserate with what we deliver here's a crazy statistic I just saw it posted again today on LinkedIn inflation since 1998 is about 82% means to make the same amount of money from 1998 till now you'd have to make about 82% more to do the same stuff during that time physician wages have dropped 10% so I'm not real good at math but I know that equation doesn't work and yet all my expenses have gone up over the years my rent how much I have to pay my team the tens of thousands of dollars to electronic health records never mind the fact that a third of the time I deliver services and see zero dollars Wow how many of your restaurant owners could keep the doors open if a third of the checks never were paid zero yeah but yet we just expect quote the greedy doctors to just keep going in this broken system and definitely and oh spend as much time with me even though we'll be lucky if UnitedHealthcare pays us $60 and 24 cents for the hour I spent with you today and the hour after our visit to send in your prescription and battle the pharmacy so what I was talking to my wife about the other day as I said let me tell you about this Pizona empty thing
17:02it's really cool and I because I had talked to you previously at our Bulls in business happy hour at Maribol every Thursday night from 5 to 7 and she said you know it's the best thing in the world when doctors do that because now they can do full panels and they can actually spend the time with you instead of having to go well insurance will do this and they they're doing your care based around what insurance will let you do and now you can genuinely have a patient come in and go all right man let's figure it out what what's going on and you can spend that time and do way more because you're not beholden to an insurance company am I off base or off on base no you're 100% correct it's it's fun again yeah healthcare you know at most doctors go into medicine with good intentions I've met very few jerks over my career unfortunately when you're in a broken system in industry for decades and I'm sure this probably resonates with a lot of chefs you can change you can become a bitter old you sob who all of a sudden is just pissed off at the world all day and it's no different in health care I've got this great patient his name is Jean and Jean comes in to see me and I make a special occasion just to come in and take care of him because we have such a good relationship I sit down and talk with him for an hour about the health concerns of his daughter would he and his wife are dealing with with intimacy and so on and he owes Joe I'm so sorry I took up all your time I said Jean I'm all yours I don't have to you don't have to apologize I don't have a waiting room full of patients because I've got to keep the volume going just to keep the lights on I can charge you a fair amount for my expertise and then you've got me I've taken walks with patients during clinic visits I said you know it's a beautiful day why do we have to sit here and talk let's go outside let's practice what we preach and move our bodies so I've taken a 45-minute office visit just walking around the block that's amazing see that that's what I would like hell yeah let's go yeah I love to do meetings like on the white or red trail at Percy Warner if you like
19:05you want to meet him like how about 7 a.m. and we'll start at the steps in Belle Meade and let's just do you know an hour on the trail together and no interruptions we're getting exercise we're not sitting in a room you're out seeing nature but it's like it's a good it's a good moment yeah no I mean I've got a colleague he refuses to take meetings with someone anymore over lunch no no offense to the restaurant owners it's only he will only go on a hike with you I think it's brilliant I mean we spent all day sitting on our butt like get out and move get out and move so that's a great segue when you're talking to and I know you've changed the name for you know HIPAA that the guy's name really isn't Jean of course not of course not so let's let's talk about what you would sit down and talk to Jean about and because I think this is there's a fear as a man I'm a 46 year old man who has never had his prostate checked right there's this fear that you're gonna go in and some doctor is gonna put his finger somewhere you don't want him to put it and I just don't know if that's not my reason I just it's finding the right doctor to go do it what what are you doing when somebody comes in what are the top three things that people are coming in for I think the main thing is people number one they don't want to feel better right I know it's a very vague thing but you just you want to feel well and I'm in my mid to late 40s as well and you know you don't have that get-up-and-go used to have you know that fire in your belly in your 20s where you could be up all night drinking going out to a late dinner and wake up the next morning and people like dude how are you moving I'm like I feel fine you know you're at least faking it you can a lot better when you're our age you go I think I'm gonna die today no exactly you know people know like I don't do dinners past 530 so you know I think that's number one is just wanting to feel better whether it's more energy more stamina just even sort of mental clarity I think number two a lot of what I do is in the reassurance business I
21:05want to make sure I'm not dying we're at an age where people around us are starting to pass whether it's our aging parents or we start hearing more and more stories of like did you hear that guy from college who's no longer with us yeah like you don't hear that very much other than awful you know traumas and tragedies in your 20s and 30s you get into your late 40s it's happening more often it is it's a thing you know in fact there was a chef famous chef Anne Burrell who just recently died age 55 I'm like she's only seven years older than me like I'm planning on being around a while you know but you just don't know unless you really go and look and then I think there's the last bucket of people who are coming in and saying okay I feel good I don't need any immediate help I'm not worried I want to optimize and those are the folks who are really looking at all the biohacks and the longevity and hey listen there's a lot of information out there a lot of misinformation help me sort through all this and come up with an actionable plan what are you doing to optimize your health what are you doing to make sure you don't have seven years but another hopefully 50 to go so I think those are the three biggest buckets now obviously as a urologist yes I'm gonna screen men for prostate cancer as well here's a good news Brandon number one pick a urologist with small hands as he as he puts his hands up in the air number two you probably don't need the finger until at least age 50 but you could get a prostate blood test called a PSA which is just a simple my dad had surprised prostate cancer so there's it if it's genetic then I should probably go get that tested earlier than 50 maybe 100% if you have a family history I'd recommend and even I don't have a family history but I've had a baseline PSA in my 40s and if it's low enough evaluated by urologists then we can often even skip that finger exam in fact a lot of the finger exams are going by the wayside so most of the time I evaluate
23:07guys I don't have to do the old finger in the butt because I know if your prostate blood test is good you're not having urine symptoms it's probably not gonna add anything so reassure guys like I'm good brother yeah but if you do think if I do think you need it again I do have small fingers for what it's worth which is a good thing I think most men you know one of the things I've been talking about is kind of that third bucket right of people who are proactively going in I feel great but I also want to continue to feel great I feel like most people utilize a doctor like yourself when things stop working whether it's in the bedroom or where in the bathroom or the boardroom when things stop working they go oh this is broken and then they come to you to go hey why is this broken can you fix it and I think that is where I want to spend a little bit of time today because I think that while we feel well identifying what you can do to not break things ahead of time means you have to fix them then when people come in to fix them what are some stats around Viagra and these other pills and what does that mean because that's when people just come in and ask for Viagra I think there's other symptoms there right you can say hey well let's look and see why you need Viagra there's a lot of stuff going on there let's start with men coming in because things are broken yeah lots on pack but I'm trying to shift more towards proactive health versus reactive health I think another way we see it is sick care versus well care insurance care versus concierge care you know that's a lot of the primary differences in ways we break that down into different buckets because an insurance company is about let's bundle a bunch of symptoms put them under a diagnosis which has a code so you submit a bill and then we pay you and what's incentivized in that system is to see as many people as quickly as possible it's a volume game and if you're a surgeon you are incentivized to do more stuff to those

29:16the demise of all of us listening to this are because of lifestyle it is not because you quote got bad luck it is not because mom and dad gave you bad genetics it is because you are putting the wrong food in your body you're not moving your body enough you are stressed you are not sleeping you're not optimizing your hormones you're not being proactive about your health and so that gives us a huge opportunity to shift the paradigm and that is what we're trying to do and why we have gone out of insurance because none of that is incentivized in the insurance model and here's the other frustrating thing we don't learn any of that medical school medical school is funded by big pharma all of these food conglomerates I mean there I just saw some study today it was released it was on LinkedIn again some nutrition expert released some study from Tufts University some top University and it talked about foods we should encourage people to eat more and at near the top of the list was frosted mini-weeds honey nut Cheerios and chocolate covered almonds whereas eggs were about half the nutritional value according to this one chart then frosted flakes frosted mini-weeds frosted mini-weeds because you know it's got wheat in it Brandon so again take that with a grain of salt I love frosted mini-weeds but they are delicious I haven't done my due diligence on it but the point is even the quote experts you can't trust them I mean if you look at most of the recommendations from the RDA they're garbage I mean there's no other way to put it I recently ran into some company formed by some nutritionists who were trying to provide healthy meals for people in rural communities and I said oh where do you get your you know how do you figure out how to make these meals like oh well we follow the RDA and I just start shaking my head I looked up their company on a website RDA is an acronym for yeah so you're recommended
31:16daily allowance and what's what's made by the federal government and I'm not a conspiracy theorist just for those listening but I looked at some of their meals and you know one of the meals they recommended someone type 2 diabetes was a meatball sub now I'm Italian from New York I love a good meatball sub chicken parm plant parm you know any cold cut hero I'm gonna destroy it that's the last thing you should be eating if you have type 2 diabetes and yet they're quote experts and they're well intended and so this brings up my point a long winded way that even well intended doctors and nutritionists and quote experts are very very misinformed and that's because of who's running those studies it's due to a lack of education and most of honestly what I've learned in the wellness space has been self-taught I've had to learn this myself over the last 10 years and I did it selfishly because I wanted to improve my health because when I looked around even at my colleagues going who do I go to for a doctor because I'd go in they check some basic labs and go you're fine I don't want to be fine I get that most of the time when I would go to the primary care will be you're young you're fine you're in shape like no worries I mean I would we would do labs I do my annual thing and they would never call me there would never be a hey this is what's going on with you it's oh didn't you look in the portal I'm like I don't know how to read that stuff I'm I don't know what this means oh it's all in there like look in the portal like I would like to have you explain to me what I'm doing or give me some advice or something that might say hey looks like you're eating a lot of this or this this number is a little high this number is great this numbers it's you're fine nothing nothing sticking out nothing nothing's crazy and it's like okay I mean I guess but I had McDonald's three times last week like what it what's going on like and I don't that's the thing we talked about this I think the
33:17other night Brandon if this were a restaurant experience you'd have a one Yelp or a one-star Yelp review yeah and good luck keeping the doors open it's it's amazing we accept that mediocrity and again I I obsess over the patient experience we don't always hit a home run but that's what we aim for but I also have deep empathy for that doctor that you saw in that system because they are overworked they're doing more button-clicking and bureaucracy than you can imagine for someone with ten plus years of schooling after high school being paid ultimately if you look per hour a lot less than people in less lucrative fields and they don't have any incentive to sit there and spend that time with you because guess what they got 40 other people and those 40 people are sick as heck and so the easiest lowest friction thing is to be like dude Brandon I don't have time for this like I really don't it's it's either spend extra ten minutes with you or get home with my kids and I'm getting home with my kids it makes sense it doesn't I don't apologize for him but I understand where that comes from and that's exactly why we're doing what we're doing because it's the system that's broken it's not that doctor they are in a bad system got it if you're a man and you want to come in because maybe you are having erectile dysfunction let's talk about this topic for a minute I mean I don't know who's listening if there's if there's a woman who's listening who has a husband who has this or boyfriend or whatever what's the most common thing that you see when men come in for this you said 80% of these things are preventable and all this stuff what's your typical take on erectile dysfunction I don't know if that's the right question to ask you know where I'm getting at yes absolutely so let's first talk how common it is 50% of men by age 50 are gonna have erectile dysfunction Wow really by age 60 it's up to 60% and you keep climbing by decade
35:18and add another 10% so it is incredibly common and why is that so the primary cause of erectile dysfunction as we age is atherosclerosis that should sound familiar people because that is the same disease process hardening of the arteries that leads to heart attacks and strokes and the arteries going to the penis and male genitalia are some of the smallest in the body that go to an organ that we see on a daily basis so if you've got a one millimeter artery feeding penis to the blood or blood to the penis blood to the penis you know what I mean hard in urology not to make some dirty pun or screw things up here but it doesn't take much of a blockage to cause the you know your man down there not be as firm as he once was you know that typically starts in in a man's 40s where he goes yeah it's not quite like it was maybe I'm not getting the morning wood I used to that's the exact time that you need to go get checked out because it is really the canary in the coal mine and that if you look at most men with Edie when they develop it they're at a higher risk for a heart attack within the next five years really absolutely so we have to break this stigma of ha ha he's got Edie or oivd I'm embarrassed or oh Edie is kind of cool I get some cool blue chew from from sexy woman on the on the TV commercial now he's gonna send me pills and you know find me attractive it's a serious medical issue now there are certainly other things that cause Edie you know most the time a 22 year old who comes in with it he's not gonna die of a heart attack he's just got a lot of anxiety and stress might be engaging in some extracurricular substances that he probably shouldn't could be contributing but certainly you know you start getting in your 30s 40s or 50s or it's associated with a waistline that hangs out more than your man does
37:20could be a sign of a serious problem and you can address that I mean if I come in like and if and this is where I was talking like you can get a blue pill and that might help but what does that do what does a blue pill do does it open up that artery how does it work yeah great question so you know the pills are not going to reverse any of that damage but they're certainly going to get you that relief in most cases says that you and your partner can get intimate again the bedroom and get that sparked up again they are what's called vasodilators fancy doctor term for dilating blood vessels so if the blood vessel is a little bit narrowed let's open it back up now for a lot of younger guys we are seeing a daily low dose of the Tadalafel or generic Cialis as a nice alternative because with that daily pill and so the blue pill you don't have to think about this you just take your ED pill every day and it can kind of dilate things open and gives you that sort of spontaneity now there's potential other benefits to taking that pill in fact I'm taking a daily Tadalafel okay and the main reason I'm taking it is number one it can help symptoms from an enlarged prostate so in my 40s I started noticing having a little bit more dribbling a little bit more waking up at night to go the bathroom and I was like you know this is pretty annoying and I didn't like the side effects of some of the other prostate medicines I also like the fact that there's some data for the daily Tadalafel to potentially prevent ED from coming on board and keeping all the tissues down there healthy so there's a bit of a you know use it or lose it phenomenon anyone who hasn't who hasn't worked out for a period of time after previously being in the gym you know things start shrinking down and we look at grandpa and you know he's not quite as muscular as dad who's not quite as muscular as you so there's a loss of muscle everywhere else in the body well down there's no different and I hate to break it to you guys it can shrink down
39:24there and so if I can take a pill that's gonna keep it from shrinking potentially because I'm getting good blood flow help maintain and then lastly there's another potential longevity benefit because it's dilating blood vessels it might help decrease the risk of heart attack stroke and dementia which are all related to narrowed blood vessels to a degree so retrospective studies we don't have any proof please don't take this and say hey dr. Pozono said this is gonna keep me alive longer I want to take some Cialis but there's enough data to say that when a man now comes in who's a little bit younger I start having that conversation proactively and I share my story with them and I want to normalize that idea like yes I'm on an ED pill is it cuz I have ED no but I also don't want to get it and that's okay so if I come in and my waistline is longer than my my member and I'm having some issues and are you going to say yeah here's the blue pill what is your recommendation gonna be for somebody who's kind of not working out and is out of shape and you can tell this is probably because you have arteries that are clogged what's the what's the remedy there yeah so this is exactly why we changed our model to go away from insurance so I had the bandwidth and the opportunity now to sit down with that guy and go okay I got a couple different pills they're gonna help you and your partner be happy in the short term but let me start teaching you why this is a big deal so I just go through what I told shared with all your listeners right now I go through the why why is this happening I go through why this is important and then I start working with them on what I call the the pillars of wellness to start identifying what are the opportunities for us to start improving your health because if you don't start changing your behaviors this Edie's not only gonna get worse you're gonna at risk for a heart attack a stroke and early death so we sit down and do a detailed deep dive
41:24what are you eating what are your trouble foods how many hours a night are you sleeping do you move your body at all if so tell me about it no that's not movement that's called walking tell me do you lift weights no okay we got to get going on that let's talk about lifting weights so good so several reasons number one the more muscle mass you have the higher basal metabolic rate you get so in other words you can get away with eating more should grab your listeners attention I know it does for me you know with eating more because you have more muscle mass and very often we we ignore that and think the only way towards health is just cardio cardio cardio and in fact probably the best way to lose weight is lifting weight so that that's number one number two it's a natural testosterone booster you can't tell me that after a good pump you don't feel nice and energized in a good mood and you know jacked baby absolutely I mean nothing gets me and my wife more frisky than you know both of us finishing a workout three we tend to ignore and this is a big part of longevity that does get ignored in a lot of cases musculoskeletal health so I know plenty of people maximize for heart health they they eat well they might walk or do a lot of cardio they're not overweight but they have zero muscle mass as we age again this kind of rule of ten every decade you start losing about 10% of your muscle mass so before you know it you might have a very healthy heart no cancer but you're so damn frail that your hips are shot you can't get out of bed and you can't lift up your grandkids so maximizing muscle gain and health and strength early on or in middle age will help stave off frailty hip fractures and other things that are big deals I mean hip fractures are a huge killer of people over the age of 65 so in fact I believe the rate is up it's a third to 50% don't quote me exactly if people after the age of 65 if they have a hip fracture will be dead at the end of the year I mean that's how
43:27serious a hip fracture is in grandma so we got to get them lifting weights along those lines how much weights do I have to lift I mean I personally me I'm like a four-time-a-week gym and I do heavy weights and I do that is that good or do I need to do five six days a week is I typically do weights for about 30 minutes and I always do about 10 to 15 minutes of cardio as a warm-up am I doing the right thing what am I doing yeah no that's a great question Brandon so I think it's a little challenging and potentially dangerous to say hey just on the fly yes that's right for you what we've got to do is when we evaluate someone before we give them a workout program is check what are you doing now because if you're doing nothing maybe four days a week of heavy weightlifting is not the right answer for you if you're a hundred and fifty pounds over weight we probably just need to get you walking and drop fifty pounds first before we talk about doing any weights if you're a 72 year old woman the weightlifting regimen is gonna be different than the 43 year old guy who's trying to maximize longevity so what we try and do at our practice is really individualize everything sure there's some core principles and beliefs I try and instill and keep consistent but we've got to personalize it and we also look at your lifestyle you know it's I think it's arrogant when doctors just go all right man you gotta eat healthy start working out see in three months okay that's great but I'm a truck driver and I'm on the road three weeks out of the month driving for 12 14 hours where do you want me to find healthy food and lift weights it's really hard to find healthy food on the road yeah so you know what we try and do it really what we try and do is find easy wins what are some easy wins we can get for you to build some momentum because I think we spend a little bit too much time in health care and especially now in this longevity space on the ideal and not enough on the best alternative solution

51:34that that's not happening well I feel like I'm like a Great Dane and you can tell me if this is true or not there's just something that voice kind of you don't see a lot of 90 year old six foot six guys I think that the bigger dude you are like the less time you have I always feel like the big huge guys never make it that long it's always I don't know why you know great Danes are they don't live that long is there any truth to that but I've always just kind of been like I don't want my wife to be a widow when I'm 60 you know or 65 I want to be that 90 year old giant no it's a great it's a great question I can't say I've seen any data on height specifically and longevity I mean if I'm just speculating and shooting from the hip I mean I imagine as a taller guy you can probably get away with more unhealthy behaviors longer because you can kind of hide it you have a higher metabolism to a degree and so you know I'm just speculating but yeah I mean the the key I probably do get away with in a restaurant you know I mean I don't eat the best well listen I'm also I love food I love cooking I love going out to eat I think Nashville's got some fantastic restaurants I'm excited every day when one opens I'm on Yelp every day looking to see what's going you got to show yourself some grace and the key is what do you do 80% of the time do you set yourself up for success by having good systems in place because listen healthy people don't have more discipline healthy people have better systems we all have cravings and that's okay so when you want to go to Bill sandwich palace and get one of their delicious sandwiches go get it and enjoy the hell out of it and I do yeah and they do that too over there yes I'm looking across from them I don't have any sponsorship deals or anything with them yet so let me ask you this if I'm overweight there is an opportunity to go walk and work out and eat right but now
53:34everybody seems to be getting thin on this thing called Ozempic there is it a GL one what what are they called yeah the GLP ones GLP ones there's other versions of that it was originally a diabetes drug correct right and so the side effect is that you lose a lot of weight and everybody's losing weight everybody's you can get these now do you prescribe stuff like that we do you do what are that what is that let's talk about that because I think a lot of people have this in their mind that they go oh well I'll just take this I'll just go get the shot and then I'll just be thin and I think there's a lot that happens when you take that shot and you don't like I think the workout regimen you have to change your life anyway you have to go work out eat lots of protein because that will make your bones brittle like what happens can you talk about Ozempic and the whole around that yeah absolutely GLP ones so the GLP ones is a general category so you're gonna hear the name brands of Ozempic will go v manjaro the generic names of semaglutide or some call it semaglutide and tersepid the medications work by slowing the emptying of food from your stomach as well as the slowing of food throughout your gastrointestinal tract it seems to also have an effect centrally on the brain so it's that you get fuller quicker so I've used it myself for a period of time I like to dabble in different things and see how they affect me and basically what you know I love that by the way I'm gonna interrupt you but like that's you've tried it look I've done I know what it's gonna make you feel like yeah absolutely and everyone's different I mean I've got for some people like awful side effects nausea heartburn can't tolerate it for a week and then I've got people like this stuff is gold I mean it's it's changed my life I in general look at it as a tool and that is probably a little bit different than the two the two Polars that I see one is the just give me the drug I don't want to
55:41change a thing and thank you good day in fact I had a guy once he came and he's like listen I want to eat what I want drink what I want not be a fat ass I want the pill and I was like yeah man it's just not gonna work appreciate the candor but exactly it's like no that's not how health works and on the other side you get the people who are like no I want to go natural and I'm weak if I do that I can do this with discipline and I'll just try my 45th diet this year I think both are dangerous extremes so I really look at it as it is a tool it is a medicine that will slow down the emptying of food from your stomach so when I took it back to my experience you could order a burger a beer and fries you eat maybe three quarters of the burger few fries half your beer and you're like I'm good I enjoyed it I don't want anymore I'm full right whereas in the past you're like the burgers gone you're eating the fries off your kids plate and beer 2 is already halfway done so it does allow you to not worry as much about what you eat because it's gonna cut down on portion control because listen to lose weight it's simple math you have to burn more calories than you consume and you can do that a number of ways you can limit portions which is probably the hardest thing for most people do you can limit food groups so this is where you've got the vegans versus the carnivores the Keto's and all that or you can limit the time there's the time restricted eating intermittent fasting sort of regimen so where you can just really burn a lot of calories yes yes absolutely but as they always say you can't outrun your mouth so there's always a way to overcome it so those are really the three main ways and so this works on the hardest one which is the portion so therefore you can eat any time of the day you want you're hungry but you just won't be hungry as much you can order the food you want but you're not gonna eat as much of it but you bring up an excellent negative downside if you don't change your
57:43behaviors and work on those other pillars you're gonna be dependent upon and when and if you stop it you might have a rebound weight gain that's even higher than where you started so that is why I look at it as a tool a very effective tool against the fight against obesity and listen if I had to pick one poison I'll pick this medicine seven days a week and twice on Sunday versus the trend our country's headed toward where today in the US 40% of adults are obese and obesity is the leading risk factor for most of the major killers from heart disease strokes diabetes even people who died from COVID 75% of people died from COVID were obese so we have to we're not talking about beauty we're not talking about fat shaming we are talking about obesity is a health epidemic in our country that we are absolutely ignoring and we're just normalizing it that I think is very potentially dangerous so if you got to take a shot so you're not obese by all means like get your crazy ozempic face like whatever it's still better than the than the alternative but I really try and look at what we're doing as that responsible middleman where we use it as a tool again we individualize it we customize our doses a problem with a lot of the name-brand medicines or the stuff you're gonna get via the online pharmacies there's gonna ship you some stuff and say good luck and you're gonna get two doses two doses the high dose or the medium dose we can personalize the dose from week to week from person to person and really get it so it's the right amount for your body and I want you on the least amount necessary to get the best results this is all really good stuff and I hope that you know if you're out there listening to this and you know I know we've got kind of granular on this if you're not a man hearing this hopefully if you have a man that you love I mean sharing this information is something I've wanted to
59:45do so bad so thank you so much for being here let's talk about testosterone yes I don't know the first damn thing about testosterone low T everybody talks about same kind of thing this brand hems you can I get these things on my Instagram I guess they know how old I am and they're like hey you can have high testosterone I think there's all these different models and athletes that are like you need testosterone what's cool what's going on here with testosterone and low T and all this stuff well I think two general comments before I deep dive number one everything we're talking about also pertains to women including what we're about to talk about with testosterone testosterone is actually more prevalent in a woman than estrogen is really absolutely but no one talks about it number two I look at testosterone just how I described this we've got the two ends of the spectrum right we've got the one end which is the traditional doctor and medical community of I'm only gonna prescribe this if you feel absolutely miserable and your levels are in the toilet and even that I'm gonna barely give you enough to make a difference otherwise get out of my office don't bother me this stuff's quote dangerous and this coming from doctors on the other end we've got the bros and bro science where you've got your strip mall center you've got your jacked dude who could doesn't even have a neck who's talking about testosterone like and I don't want to look like that and that's not healthy again we try and be that ethical moral middle ground that individualizes it so here here's the lowdown testosterone is a hormone made in men and women mainly in the gonads that means the ovaries for women and the testicles for guys we do have adrenal glands that make a little bit of them but for the most part think of it as you know the the boys down there making testosterone sure testosterone has effects from the top of your head to the
01:01:46tip down to your toes everyone your body testosterone is playing a role hair bone loss your mental acuity and sharpness your muscle mass your heart health your prostate health your intestinal tract everything is affected by testosterone and again this goes for men and women so it's not just the libido hormone although that does play a big role it's not just the I want to get jacked hormone although it can help you put on muscle mass and we've got data showing that even if you fall in specifically now talking about men in the quote normal range which runs anywhere from about 250 to 800 as a total number just to kind of give you a framework if your levels are below 600 which most would be like well 600 is kind of like on the high side sure if you're below 600 we have studies suggesting a higher cause mortality what does that mean if your t levels aren't above 600 you're the higher likelihood for dying from everything so it is not just the bro get jacked hormone it's the longevity hormone so what we try and do again is number one i'm a big believer in data getting your hormone levels checked is never the wrong answer the key part of the equation where it usually goes downhill is getting in front of the expert who's not always incentivized to just push it on you at all costs so that's where again we sit down and have a personalized discussion with a man this can happen as early as a guy in his in his 20s i have guys in their 20s with testosterone of that of a guy in his 80s and then i've got older guys who are on it so we look at your number we do assessment of your symptoms how are you feeling and then we make a decision are we going to go all natural and really on your diet get you on some supplements get you lifting weights maximize your sleep here's a big thing and i'm sure for a lot of your listeners who probably struggle with shift work and not not sleeping

01:05:52me but there's a lot of men who don't want to talk about this stuff who there's their shame there's there's mental health issues and i know you're not a mental health professional we haven't even got into mental health yet but how can we reframe the idea of taking care of your health as weak or unmanly like these just the stigma that oh man i'm a man's man i don't need that shit like i can do whatever you know i'm fucked that it's like no man you need 80 percent of of things that you're going to die from are preventable that's a very crude way of saying what you said earlier twice so how can we frame reframe that idea that mental or that health care mental health is weak or unmanly i think it starts with what we're doing today and i applaud you for bringing on a physician to a restaurant podcast to talk about this because we do need to normalize the conversation so step one is we got to build awareness and i love when i hear patients who come in and see me who talk about this with their families with their loved ones with people in their church group and guys they go to play golf with and people at their a.a meetings whatever it is whatever organization you're a part of you got to talk about this because whereas i think culturally women get together in groups and talk about everything guys get together and talk about nothing we're going to talk about you know who's going to win game six tonight of the nba finals we're going to talk about you know who's going to go first in in the nba draft we're going to talk about the classic meme where it's like the the husband comes home from playing golf with ted and she's like so how's ted doing he's like he's good like i heard him and martha are breaking up it's like yeah this is what happened i don't know like i was within the whole day we didn't talk about it at all like this isn't what guys this is just we played golf all day what am i supposed to know what happened or anything i don't know yeah and i think that transition happens in our 40s right i've we just went and saw some stand-up
01:07:56over at the ryman it was great this weekend and all the comics were in their 40s and they talked about this yeah in your 40s you have a watch because you got to know when bedtime is in your 40s you start talking about health and you know what's hurting right now and what longevity hack you're doing and so i think that's the biggest thing there's no secret sauce to this we just have to talk more about it and i've heard so many stories of guys when they heard someone they trusted went to the doctor how impactful that was we just we dropped a podcast recently um with uh with my business partner and he shared his story of going through a low t journey and he was miserable but what changed it was when one day he's working out with his buddy and his buddy was like dude what's up something's wrong and he then shared with them what was going on in his life and his body's like dude you need your t-levels checked i'm on it and it's like changed my life we need more of those conversations and i love what you're doing here because a podcast isn't one-to-one it's one too many yeah and so i think we just need to keep promoting and doing things like this and uh you know if any of you need to learn more about this and further spread the bullhorn you know come check out our podcast and and listen because this is the stuff we talk about every single week yes his podcast is called balls deep and you can find it on apple podcasts everywhere podcasts are yeah i'd say youtube is our preferred channel just because uh we got a great video production uh guy chad and just want to give him a shout out and get him more business but uh we're also pretty funny and i'm italian i speak with my hands so you know you may not get everything i'm saying if you don't see me he's telling you to go to youtube and search balls deep and not get in trouble and your wife sees your search history that you're typing in balls deep into youtube well maybe you should just type in pazona p-a-z-o-n-a there you go and then balls deep will come up you know we all have our workarounds with our google searches um one
01:09:57question then i want to do some plugs i want people to learn i want to learn about what you do and i know you got to get going here um if there's one thing you wish every man would do for himself this month one thing that you wish every man would do for himself this month what would it be schedule an appointment you just got to pick up the phone and go and do it because you you can't drive a performance car without a team and that's what you want to build you want your body to be a performance sports car you don't want to be a ford pinto and you can't do it alone because even myself as someone with 20 plus years of medical knowledge i get confused i'm doing what i'm doing because i looked at the landscape i was like i don't know what the hell to do i'm in my 40s i want to feel better where do i go it's not the traditional system because i know more than my primary care colleagues in some cases because again they're really good at illness that's all they've ever been trained on it's all they ever do on a day-to-day basis the guidelines are are completely out of date and inaccurate so you got to find someone that you can trust that you can sit down with have an authentic conversation get some data and develop a plan so that's the one action item you can take is just pick up that phone make a phone call get in and see someone because listen you can have all the money in the world you can have the best relationships in the world if you don't have your health you have nothing and whether you want to pay today or later you're going to pay and i'd rather pay now by putting good food in my body getting my blood drawn sleeping eight hours checking my hormones and live a long healthy fruitful life than the alternative of paying later by sitting all day in a doctor's office waiting for my type 2 diabetes medicine getting another heart catheterization undergoing another surgery losing my mind and my ability to interact with my family that's just not in the table for me

01:14:01and it's lots of fun and doctors don't know anything about it it's weird how much they don't know anything about it you talk about full hormone panels and looking at all those things that has a lot to do with perimenopause from what i understand and most doctors will just kind of they just gloss it over and i have a checklist and you're fine or here take this take this do you see women do you absolutely do you see women for that sort of a thing because non-insurance providers and perimenopause is a terrible thing seeing somebody who's genuinely going to learn what your hormones are at and can create you know a roll-on or a pill or whatever it is that you know is perfect for that individual person that is something that you do too we're a destination for couples now it doesn't mean if you're not in a relationship we're not going to see you but we can see men and women and very often we are seeing couples together because health thrives when the family vibes you've got to get on the same page as your partner and if you're trying to do healthful things and your partner isn't or even if your partner wants to but it's just not going to the right person you know i can i can help guys get erections all day if i don't help their their female partner get in the mood and deal with perimenopause menopausal symptoms ain't going to use it yeah or he does he's in trouble so we could do a whole other episode but yes i want your listeners to leave that although the focus for this with men's health month and mental health month was was the guy listen we know women drive the health care decisions in families and are a often marginalized portion of our medical society and practices and that a lot of health care is built for the the man but we absolutely take care of women and address their hormones and can help with weight loss and even urinary incontinence and other women's issues we haven't talked about today maybe we could do a follow-up episode but i would love that i would love to do a follow-up episode and bring my wife on she can
